pull the stops that hold the window in place, clear away all the glazing, and install new glass, and then reglaze it. Bucky at Hadley's used to do that. Not sure who might do it, if you can't or don't want to. Try Judy's mirror and glass, or maybe Nugents.
Ok don't laugh but how do I pull the stops and clear the glazing?
Depending on the condition of the glazing you may be able to pull it off with your fingers. CARFULL OF BROKEN GLASS. But if not a utility knife(razor blade), or a putty knife will work depending on how well the glazing is still stuck to your wooden window sash. carefull not to remove or shave off too much wood from the window. We only want to remove the glazing. The proscess of removing the old glazing usualy pulls out the old glazers points (AKA stops) too.

You can pry them out with just about anything. I would recommend needlenose plyers to pull them out. But a flat bladed screwdriver works easily to pry them out.
